The purpose of downloading the Agents of Slabs form is for when you’re mailing your books to us, we have your information to contact you. Customers will need to fill out the demographics section of the form and list their books in the Item Description section. Customers do not need to fill out the price or totals as we will do that when we contact the customer to finalize payment.
